 The format of the questionnaire will be the same as in the housing plan questionnaire, with a few minor exceptions. The questionnaire must be completed in English if English is not the primary language of the units being considered. The HUD program or project (e.g., HUD subsidized housing, rental subsidies, etc.) must be identified in the housing plan (FHA Program) that is being developed, or the HUD programs that may be used with the proposed project (e.g., HUD-related subsidies, tax exemptions, etc.). For a HUD-related subsidy, this questionnaire should be submitted in the form of an exhibit which will be attached to the FHA Program's Rental Application(s). For a tax exemption, the questionnaire is attached to the tax exemption application. Please provide a summary of the criteria and criteria for the proposed project, including a list of the projected characteristics and values used to evaluate the viability of the project in terms of housing affordability criteria. Please provide information on the HUD programs that will be used to evaluate the proposed project, including an evaluation of how these programs will provide the following benefits:   (a) a specific housing opportunity for high-income families;  (b) a specific housing opportunity (i.e., one that includes rental units, affordable private rental apartment complexes, multi-unit developments, or similar housing types) for families with incomes up to 40 percent of Area Median Income (AMI); and  (c) a housing opportunity for families with incomes up to 80 percent of AMI, provided that the proposed project will include 1 or more of these multifamily housing units. Please indicate whether such programs exist where your proposed project may be located. Please provide detailed criteria for making such a determination (e.g., housing types, type and location of structures, types of construction types, the age of the structures, etc.). Please provide a description indicating whether the proposed project will include (i) the use of one or more affordable housing units, (ii) the use of a public transit system that provides access to the public transportation system, (iii) the use of any other housing opportunities, including other types of housing, which are designated for low- and moderate- income households (e.g, Section 8/ETC) in the region under consideration.
