Amount of Workers Reported by Employer For Each of the 4 Centennial Periods. This report provides employers with the data showing the total number of full-time employees (including employees not at work but eligible to be paid at regular and overtime rates and employees not at work and not eligible to be paid at regular and overtime rates) for the 4 quarterly periods ending January 1, 2015, October 1, 2014, June 1, 2013, and December 1, 2012. It provides employers with data for those employees who worked an average of at least 20 hours in any quarter of the fourth quarter of 2012, at least 55 hours in one quarter in 2010, at least 40 hours in one quarter in 2009, and at least 55 hours in one quarter in 2007. 5. Business Name. Businesses with four or more employees, and for whom the employer earned wages or who reported wages for at least 50 full-time employees, and who paid at least 500.00 in wages, are required to file an Employer's Employer Identification Number (EIN). Employers should retain this number for five years after the end of the period covered by the return. An EIN is not required where more than two taxpayers use the same name, trade name, or business address. 6.
