Employer and Occupational Injury Investigation Report. All workers should read this report closely as it may affect their future health. The report includes an outline or a summary of all injuries and diseases, medical bills, prescriptions, and other medical data, from which an estimate of the amount of compensation might be derived. The report should be submitted to the Division of Labor Statistics. The employee should be advised in a concise, easy-to-read manner of the procedure for obtaining the report or for requesting a copy from the Division of Labor Statistics. Labor Standards Code 1.60.060(2) Employers must furnish written information to their employees as to the amount of compensation to which they are entitled for occupational injuries. If an employee has not received the information referred to above, an employer may, at any time, report to the Division of Labor Statistics the cause of the injury or diseases, whether accidental or not, that the employee has sustained, or the nature of the injury or disease. When reporting injuries or diseases, employers should furnish a brief summary of the injury or disease and the nature of the injury or disease. Each patient, from whom information is requested by the Division of Labor Statistics, must be examined. NOTE: If the employer's medical knowledge is inadequate, it must, upon request, refer the case to its authorized representative for consultation and, if necessary, report its failure in writing to the Division of Labor Statistics.
