the service is great, however the payment option is miss leading, as it says $6/month and when you go to pay you charge the full year up front, in US$ and some of us are poor, so thanks for that.

A parent who needs their signature notarized must personally appear before you. Many parents aren't familiar with notarial rules and procedures, and may ask you to notarize an absent spouse's signature, not realizing it's against the law.
In no jurisdiction would it be legal to notarize a document without the signing person present. It is impermissible to notarize the document after the person has signed. The entire point of having a notary is so that the notary can attest that the person who is alleged to have signed, actually did sign the document.
Have an acceptable form of identification. Fill out and bring the document that needs to be signed. Be ready with payment for notary fees. Ensure all parties involved in the signing are present.
Step 1 Find a Competent Guardian. Step 2 Inform the Guardian of Child's Medical Issues (if any) Step 3 Inform the Guardian of the Child's Medications. Step 4 Determine an End Date. Step 5 Sign the Document.
Patient Information Create forms that require the patient's name, phone number, address, email address, date of birth, social security number, and any other identifying information you think would be valuable. Sometimes a parent will need to release medical information on behalf of their child.
Notarizing a Letter of Consent Typically, the letter of consent must be notarized, as mentioned before. Banks usually have notaries available. Call your local bank and ask if they can help you with a notarized letter of consent. Some banks have a policy that does not allow them to notarize a non-bank document.
This ensures that you are not signing the letter under duress, and that you are in fact the person giving the authorization. In some instances, it is a good idea to have your letter notarized by a notary public. This person is someone authorized by your state to attest to legal documents.
