Who needs a DD form 370?
When applying to US Army, applicants must provide references for school and employment history. Recruitment departments must verify information about applicant’s past. In order to receive characteristics from the people listed in the application, recruiting officials must send out form DD 370.
What is DD form 370 for?
Form 370, or “Request for References”, is an essential document in the beginning of a career in US uniformed services. The main purpose of this document is to verify applicant’s identity. There must at least three character references: from a school official, previous employer and a personal reference.
Is it accompanied by other forms?
It is one of the forms in the package for a person who wants to join US Army. The person who needs to provide information about an applicant, does not have to attach any addenda. Recruiting official will add a return envelope to the form 370.
When is DD form 370 due?
The due date must be included in the letter.
How do I fill out a DD form 370?
Recruiting official must fill out a TO block with an address of the institution (unit) that requested references. Also, it is recruiter’s obligation to provide applicant identification data, such as name, mailing address, date of birth, school attendance or employment. Recruiting officer must provide his or her identification data at the bottom of page 1. Page 2 is for references. The respondent has to answer a questionnaire, write a personal narrative evaluation of the applicant and provide his own identification data.
