How do I fill out form 67-9?
This form consists of seven parts. The first part is administrative data about a person who is under evaluation. The second part contains the names, ranks and SSN of the raters. Part III is dedicated to the significant duties and responsibilities of the evaluated. Part IV is called Performance Evaluation — Professionalism. There is a detailed box where the rater can choose if the evaluated officer possesses Army values (honor, integrity, courage, loyalty, respect, selfless-service, duty), leadership skills and attributes. In part V, the rater has to decide if the officer’s performance was satisfactory and if he deserves a promotion, giving comments on specific aspects of the performance. In parts VI and VII the intermediate and senior rating officers must write their opinion about the person under evaluation. The senior officer describes whether the soldier’s potential compared to the soldiers of the same grade was outstanding or below average. Also, he decides if the soldier should be retained.
