Who needs the DL-90A form?
The DL-90A is the Texas Department of Public Safety Form officially called the Driver Education Affidavit. Each instructor who teaches a student in each phase of the driving course is required to have an affidavit. The affidavit(s) of the student’s instructor(s) must be presented when the student is applying for an Instruction Permit. An Instruction Permit is a sort of provisional driver’s license for learners that can be issued only if the person is enrolled in the driver’s learning course.

The Texas DL-90A form is necessary to provide the information about the learner’s driving instructor and confirmation that the student has taken the required amount of instruction.
When is the Texas Driver Education Affidavit due?
The Form DL-90A must be presented by the instructor when the student is applying for the Instruction Permit. If the instructor cannot be present at the moment of the student’s applying, the affidavit must get notarized before being delivered to the Drivers’ License Office.
How to fill out the Texas DL-90A form?
The DL-90A Driver Education Affidavit is a one-page form that must be filled out in order to provide the following information:
