The Alabama Child Labor Law A Class I Child Labor Certificate is required for the employment of 14 and 15 year old minors. A Class II Child Labor Certificate is required for the employment of 16 and 17 year old minors.
The term “child labour” is often defined as work that deprives children of their childhood, their potential and their dignity, and that is harmful to physical and mental development. It refers to work that: is mentally, physically, socially or morally dangerous and harmful to children; and/or.
This form is required to prove a child is physically fit for employment as a child performer. This form must be sent with the Application for an Employment Permit for a Child Performer, LS 561. This form must be completed by a licensed physician, physician assistant or nurse practitioner.

In its place, employers will be required to purchase a Certificate; Class I, to employ minors age 14 or 15 and/or Class II, to employee minors age 16 or 17. The certificate will be applied for online for a fee of $15.00 per certificate.
