Who needs a DD 2535 form?
This form is used to request the U.S. Armed Forces aircraft participation at public events in support of community relations programs and request an aerial demonstration team to perform on a military installation. The form is completed by the event sponsor and submitted to the military services.
What is the purpose of the DD 2535 form?
The Request for Military Aerial Support is used to determine the eligibility of an event for military aerial support. The event sponsor contacts all the appropriate military units and coordinates any possible participation. The form contains the information about the event and place, sponsor, program and support.
What other documents must accompany the DD 2535 form?
This form can be accompanied by other supporting documents if the military service requires it.
When is the DD 2535 form due?
This document should be completed and filed beforehand so that the event organization and military unit has enough time to prepare for the event. The estimated time for completing the request is thirty minutes.
What information should be provided in the request?
The request form has the following sections for completion:

Activity: category requested, date of event, type of the aircraft requested, military service requested

The coordinating official and the sponsor have to certify the request with their signature and date it as well.
What do I do with the form after its completion?
The completed and signed request is forwarded to the appropriate Military Service. The sponsor should retain one copy of the form for future reference.
