How do I fill out a Florida request for visiting privileges?

There are two parts in this request. First you need to read Visitor Rules, or Part 2, in order to certify that you are informed and that you agree to follow those rules. In Part 1, you need to write the inmate’s name, give his DC number and describe your relationship to the inmate. After that write your full name and contact information. Answer the questions about your employment status and background. If your visiting privileges have ever been denied, you need to explain the reason. Also circle one of the given variants to inform the officers how you got acquainted with the inmate. Sign this form only if you have read the Part 2. Do not try to visit the inmate before he or she informs you of your application approval.

Each inmate will be allowed up to two (2) visitors only per visitation day (two adults or one adult and one child). A parent or guardian must accompany children under the age of 18. All children must have adult supervision before, during, and after a visit. Presently, an inmate can only visit with 2 people at a time.
Minimum custody means the individual is eligible for placement at a community release center. Medium custody inmates are eligible for placement at a work camp with a secure perimeter. Close custody inmates must be maintained in a facility with a secure perimeter or with direct supervision outside.
You must contact the Classification Officer at the inmate's current facility (see the Statewide Facility Directory for address information). Who has final authority on visitation? The warden at each facility has the final approval or disapproval authority for each visitation request.
For Video Visitation, the friend/family must be on the inmate's approved visitation list. Video Visitation sessions can only be scheduled by friends/family, not the inmate.
The inmate you wish to visit must send you a current official application. Alternatively, you may complete an application online. The electronic application can be located on the Florida Department of Corrections Website/Offender Search/Inmate Population Information Search/Inmate Population Information Detail page.
Regular visiting is held on Saturdays and Sundays from 9:00 a.m. through 3:00 p.m. Eastern Standard Time (8:00 a.m. through 2:00 p.m. Central Standard Time). Registration begins at 8:15 a.m. (7:15 a.m. CST) on both days.
PLEASE NOTE: Each visitor 12 years of age or older must submit a visitation request form individually. Pursuant to Rule 33-601.723, Florida Administrative Code, Visiting Check-In Procedures - Only five (5) approved visitors, 12 years of age or older, at any time may visit an inmate in the visiting area.
