Using this information school authorities can provide necessary care to the student in case of a sudden medical emergency or natural disaster. It’s also very important that an emergency card contains parent's’ contact informatic, solo school representatives can get in touch with them if something happens.
Is Student Emergency Contact Card Accompanied by Other Forms?
Generally, emergency card is completed without any additional documents being attached to it. However, a student may be asked to provide more information that isn’t included in the emergency contact card.
When is Student Emergency Contact Card due?
Student emergency contact cards are filled out at the beginning of the academic year and should be updated every year.
How Do I Fill out Student Emergency Contact Card?
Student emergency contact card is a two-page form. Each page is dedicated to particular type of information. The first page contains student personal information such as:
- Name, home address, grade,
- The names of both parents, their home address and place of work
- Name and address of additional person who may pick up a student in case of emergency.
The second page includes students medical records:
- Health insurance information
- Vision and hearing conditions
- Emergency treatment, etc.
Where Do I Send Student Emergency Contact Card?
Generally, student’s basic emergency information is kept in registrar’s or admissions office. However, some schools may divide student emergency contact card into two parts and keep medical records in university health center.
