Who needs form CDD-104?
This is a form for a driver license or learner’s permit for applicants under the age of 18. Applicants need to sign it at their educational institution in order to prove eligibility for a license. It is used in Texas.
What is form CDD-104 for?
Form CDD-104 is also called Perform. VOTE stands for Verification of Enrollment. It means that the student has to prove that he is currently enrolled in the educational institution and his attendance in class is at least 90%

When is form CDD-104 due?
This form is valid for 30 days during a school year and until the first day of the next year if it was issued during the last 5 days of the current school year.
How do I fill out form CDD-104?
The student only needs to put down his name, date, and signature. The school administrator who verifies student enrollment and attendance has to give his contact number, write the issuance date and sign this form. Only after that can the Department of Public Security can accept his application. N.B. for administrators: this form is considered a governmental record in Texas Penal Code, 37.01 (2).
