The 5 Stages of Palliative Care Stage 1: Stable – Developing and Implementing the Care Plan. Stage 2: Unstable – Adjusting the Care Plan & Preparing Emotionally. Stage 3: Deteriorating – Shifting to End-of-Life-Care. Stage 4: Terminal – Symptom Management, Emotional & Spiritual Care.
The following are the nursing priorities for patients in hospice care: Provide symptom management and comfort measures. Facilitate open and honest communication. Support decision-making and advance care planning. Coordinate interdisciplinary care. Offer emotional and spiritual support. Respect cultural and religious beliefs.
Early palliative care intervention can support you to live well while managing your illness. The earlier you can access palliative care after a diagnosis of non-curable cancer, the better the support services can be set up for you, your carer(s) and family members.
The VA calls this concurrent care: you get hospice and all the treatment for your medical illness at the same time. There is the requirement that you understand that you have an incurable disease. And two physicians need to agree that you have a prognosis of six months or less—we use guidelines for that decision.
Interventions: Establish rapport with the patient and family members. Evaluate the responses of the patient to end-of-life care. Involve the patient and family in planning care. Ensure that the patient and family understand advance directives. Anticipate referral to social work or spiritual services.
As a patient approaches end-of-life care, nursing interventions include the following: Eliciting the patient's goals for care. Listening to the patient and their family members. Communicating with members of the interdisciplinary team and advocating for the patient's wishes. Managing end-of-life symptoms.
