I tried it before purchasing. The font, tick box selection and general feel is better, in my opinion, than that I have experienced with Adobe. My abilities are significantly challenged due to illness. The PDF document that I have to use presents with problems when I run it in other edit utilities. I may call on you if I get stuck, but so far I am favorably impressed with this product. Thanks. ;-)

Is Polst considered advance directive?
Differences between an advance directive and a POLST Form Unlike advance directives, a POLST summarizes the patients' wishes in the form of medical orders. An advance directive is a legal document that allows you to share your wishes with your health care team if you can't speak for yourself.
How is Polst different from DNR?
A POLST form may be used in addition toor instead ofa DNR order. Like a DNR order, a POLST tells emergency medical personnel and other medical providers whether or not to administer cardiopulmonary resuscitation (CPR) in case of emergency. ... To learn more about POLST forms, see POLST Forms in Your State.
What is the difference between CPR and DNR?
A DNR order is a request not to have cardiopulmonary resuscitation (CPR) if your heart stops or if you stop breathing. Unless instructed otherwise, hospital staff and paramedics will attempt to revive any patient whose heart has stopped or who has stopped breathing.
What is the meaning of Polst?
POLST (Provider Orders for Life-Sustaining Treatment) is an approach to improving end-of-life care in the United States, encouraging providers to speak with patients and create specific medical orders to be honored by health care workers during a medical crisis.
Is a Polst legally binding?
POLST forms can also be completed with nurse practitioners, physician assistants, social workers, and other medical professionals. The form must be signed by both the patient and the physician or medical professional. If a medical professional has not co-signed the form, it's not legally binding.
What is a do not resuscitate order?
A do-not-resuscitate order, or DNR order, is a medical order written by a doctor. It instructs health care providers not to do cardiopulmonary resuscitation (CPR) if a patient's breathing stops or if the patient's heart stops beating.
Is a copy of a Polst valid?
However, POLST on any color paper is valid. Copies and faxes of POLST are valid. The original, signed POLST travels with the patient. Whenever possible, ensure the Advance Directive (if one exists) and POLST form are consistent with each other.
How long is a Polst valid?
It is particularly useful for people who are frail and elderly, who have a life-limiting condition, who have a life expectancy of 1 year or less, and/or who have a terminal illness. POLST is also appropriate to use for anyone who wishes to further define their preferences regarding life-sustaining therapies.
Is a Polst a legal document?
A POLST Form is completed by a health care professional in conversation with the patient. Since it is a medical order it must be signed by a health care professional to be valid (which health care professional can sign varies by state).
Does a Polst expire?
POLST is not meant for younger, healthy persons. And, importantly, POLST forms do not expire meaning patients may be held to their choices many years after their preferences have changed.
