To Whom It May Concern, I need to let you know that I jumped the gun on submitting my prior severely bad review. Knoll G ended up figuring out the issue and assisted greatly in resolving it. She had much more patience than I had as I had been trying to figure out where everything had been changed to from when I used your site the year before. After 4 hours of much frustration, Knoll G saved the day. I would like to ask for you to somewhat disregard my initial review. I only say somewhat because I feel that the site had such a huge change from last year's "easy to use" formatting. I never needed to use support help. That said, I would like to praise Knoll G. She never gave up even when I did. Huge THANK YOU for not giving up Knoll! You're great and I hope you get some sort of reward for going above and beyond. I would also like to add a technical note...I don't know how everything is viewed on the companies side, but it was very difficult to work with support in terms of the chat box. Every time Knoll G gave me a direction to go and I went (which means the screen changed), I would lose the chat box and had to wait for Knoll G to type and send me something before the box would reappear and then I could respond. It only added to my then current frustration so your site coders might want to look into that.

Understanding Observation Status Observation was meant to be a short period of time for providers to assess whether patients required admission for inpatient care, or could be discharged. Typically, this was meant to last fewer than 24 hours and only rarely spanned more than 48 hours.
The best way to avoid being blindsided is to be informed. When you are told that you are being admitted to the hospital, ask the doctor if you will be an inpatient or in observation status.

You're assigned inpatient status if you have severe problems that require highly technical, skilled care. You're assigned observation status if you're not sick enough to require inpatient admission, but are too sick to get your care at your doctor's office.
Generally a person is considered to be in inpatient status if officially admitted as an inpatient with the expectation that he or she will remain at least overnight. An inpatient admission solely because the patient has been kept in observation status over 23 hours would not be considered medically necessary.

The difference between inpatient and observation status is important because Medicare pays different rates according to each status. Patients admitted under observation status are considered outpatients, even though they may stay in the hospital for several days and receive treatment in a hospital bed.
Observation services are short-term outpatient services received when you are in the hospital for monitoring purposes and/or to determine whether you should be admitted as an inpatient. Because observation services often involve an overnight stay in the hospital, they may look no different from inpatient services.
Observation is basically considered a billing method implemented by payers to decrease dollars paid to acute care hospitals for inpatient care. It pertains to admission status, not to the level of care provided in the hospital. Unfortunately, it is felt that no two payers define observation the same way.
Medicare only covers nursing home care for patients who have a 3-day inpatient hospital stay Observation Status doesn't count towards the 3-day stay. Outpatient Observation Status is paid by Medicare Part B, while inpatient hospital admissions are paid by Part A.

Length of hospital stay. The LOS refers to the average number of days that patients spend in hospital. It is generally measured by dividing the total number of days stayed by all inpatients during a year by the number of admissions or discharges. Day cases are excluded.
