Great!! Saves me lots of time where I would have to find the form print it out and then type the information. The only thing that is a problem is the instructions within the form itself. I feel the instructions throughout the form should not be on the printout itself.

Launch the Certificate Services snap-in (click Start, Programs, Administrative Tools, Certificate Authority). Navigate to your CA in the left pane, then expand its node. Click Pending Requests.
Click Start, click Run, type cert mgr. Msc, and then click OK. In MMC, expand Certificates — Current User, and then expand Personal. In the right pane, right-click and point to All tasks, and then click Request New Certificate.
Issuing certificate authorityIssuing CA's are the actual CA's used to issue certificates to computers, users, and network devices. Additionally, certificates are automatically issued when a user or computer requesting a certificate has permissions to issue new certificates.
