A certificate of creditable coverage (CCC) is a document provided by your prior insurer that indicates your insurance has ended. The document itself includes your full name, effective dates of coverage, and the cancellation date.
A Certificate of Creditable Coverage may be obtained from your former health insurance carrier. Please contact your previous health carrier and request them to provide you with a certificate. This certificate may partially or fully apply to your new coverage and alleviate pre-existing situations.
The HIPAA certificate of creditable coverage is no longer a required notice. The certificate was eliminated at the end of 2014 because its primary function was to address pre-existing condition exclusions (PCs). The ACA has since prohibited all PCs, which is why the certificate is no longer required.
Certificates of creditable coverage must be issued when a covered employee or dependent has a COBRA qualifying event or otherwise ceases to have coverage under the employer-sponsored health plan, when COBRA coverage ends, and upon request within 24 months after coverage ceases.
