The letters used on the chart are C, D, E, F, L, N, O, P, T, and Z. When taking a vision exam, one eye is covered, and you are asked to read the letters of each row aloud beginning at the top of the chart. The smallest row that you can read correctly indicates the visual acuity in the eye being tested.
This line, designated 6/6 (or 20/20), is the smallest line that a person with normal acuity can read at a distance of 6 meters. Three lines above, the letters have twice the height of those letters on the 6/6 (or 20/20 in the US) line.
The eye exam charts are not the same for all purposes. You are likely referring to the Snellen visual acuity chart. As above, the chart can be validated for illiterate people or pretenders. Obviously, there are lots of different eye charts for non-Latin-alphabet readers.
To interpret your reading, think of the numbers in feet. For instance, someone with 20/60 vision can read at 20 feet away what a person with normal vision could read at 60 feet away. Modern optometry exam rooms are equipped with electronic eye reading charts, making it easier to produce accurate readings.
Usually the 20/20 line of letters is fourth from the bottom, with 20/15, 20/10 and 20/5 below that.

On a Snellen eye chart, row's letters of a given size have the corresponding distance number next to them. Thus, row of 18 mm letters is marked 40. The font must be Couriered (or Courier Bold) in order for the “font size” in points to correspond to the indicated letter height.
