The easiest way to make a copy of a Form is through Drive. In your Drive, click once on the form you would like to copy and then open the '3 dot menus' to the right of the screen. — Once the copy is made, you may want to rename the file so that you will be able to find it more easily.
To duplicate a form you would require the permission from the form Creator. As stated it is not your form so by proper channel it is not possible. You can either contact the form Creator to provide you the access to a copy of the form or can send you the template for that from.
Just click the 3-dot button in the top right corner of your Google Form, and select Copy. You can then give the copy of the new form a different name, and tweak anything you want.
Need to make a copy of a form in Microsoft Forms? Furthermore, you can easily do that with just a few clicks. Find the form you want to copy in your list of forms, click the ellipses button () in the upper right corner of the form preview window, and then click Copy.
