What is a Notice of Election?
Medicare program that a beneficiary's election is on file by submitting a Notice of Election. (NOE). The NOE is submitted like a claim. The NOE processes through Medicare claims systems, which updates beneficiary records and later uses the information to adjudicate hospice claims.

Notice of Termination/Revocation (NOT) 8XB Hospices are required to submit a Notice of Termination/Revocation (NOT), also known as a type of bill (TOB) 8XB, within 5 calendar days after a hospice discharge/revocation, unless a final claim has already been submitted.
What does Hospice revocation Code 2 mean?

Where does the GW modifier go?

What happens when you revoke hospice?
A hospice revocation is a beneficiary's choice to no longer receive Medicare covered hospice benefits. To revoke the election of hospice care, the beneficiary/representative must give a signed written statement of revocation to the hospice. The statement must contain the effective date of the revocation.
How do you bill Medicare when a patient is on hospice?
Only an attending clinician who is not employed by the hospice can bill Medicare Part B for hospice care using the CPT E/M code. If the hospice physician serves as the attending physician, all services related to the terminal condition are billed to Medicare by the hospice, not directly by the physician.
