Its good for the most part - the downside is it doesnt open for me on the first try and it asks me to sign in everytime i need to print or save a doc. even though I pay for the service. Annoying that it doesnt recognize me when I use the app.

Easy way to fill in forms online Easy to get things filled in and returned to customers without having to print, write and then scan or fax back. Saves a lot of time in getting payment information from guests. This software doesn't really do anything else except for fill in PDF files. If this is all you need then great but it would be nice if it could do some other tasks with PDF files as well.

Common questions on how to Create Surname Field Forms

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Your name field should be culturally inclusive so that no one struggles to fill out your form. With most things in life, having two is better than one. But when it comes to name fields, having one is better than two. In this case, we need the first name to be separated.
e.g. last name, middle names, first etc. Say you want the user to enter their full name into a form. Even if you request just this, you may not know which order they put their names down (for example in countries like China, surnames come first).
The correct format to be used in all Government issued documents are First named(Given name) Middle name followed by Last name (family name or father's name). Always expand your initials and mention your full name in all the documents else you will end up with different names on different documents.
Single Full Name Field A single field labeled Full Name is more culturally inclusive. The full name may include first, middle, last, family and other given names. It allows users to type in their name without splitting them into first and last.
Set up first and last name custom fieldsClick Lists & subscribers, then select the list you want to add the custom fields to. Click Custom fields in the left menu. Click Add custom field. Under “field name”, enter First Name, set the “Data Type” as Text and click Create field. More items
