I have used the free trial twice. When I signed up for a $6 one month personal subscription I was billed $20USD. I have sent an email about this and hope to have it resolved.

The forms are very easy to fill out. I was unable to comments in a few places where I ran out of room for heirs and needed to include a comment to refer to an additional page.

Filing a claim or amending one To confirm you're eligible to file a claim, enter your last name and the last six digits of your Social Security number at www.equifaxbreachsettlement.com or call the Settlement Administrator at 1-833-759-2982. You can file online or by mail.
In July, credit bureau Equifax agreed to pay nearly $700 million over its massive 2017 data breach. Under the terms of the settlement, affected consumers could potentially get up to $20,000 in reimbursement.
The credit-reporting company has agreed to pay between $575 million and $700 million to settle state and federal investigations related to a massive security incident that exposed the personal information of more than 147 million Americans two years ago.
The settlement, unveiled in July, essentially gave consumers two choices. They could take up to 10 years of free credit monitoring. The first four years would be with Equifax and two other credit bureaus, Experian EXP GY, -6.27% and TransUnion TRUE, -4.52%. The next six years would be with Equifax.
Go to the settlement website and click on Find out if your information was impacted to see if you're eligible. Identity restoration services include help dealing with companies, government agencies and credit bureaus. You can use the service even if you never make a claim from this settlement.
Despite the FTC pushing people away from an Equifax cash payout, there's a good chance you'll get all $125. Eventually. Last week, Equifax agreed to a $575 million up to $700 million settlement over its major 2017 data breach.
Prepare and file your claim To follow the settlement, the FTC suggests you sign up to receive email updates. You can also check the FTC's Equifax Data Breach Settlement page and the official data breach settlement page for updates. And the FTC has a number you can call -- (833) 759-2982 -- for updates.
To see if you have been impacted, start by going to the Equifax data breach settlement website and enter your last name and the last six digits of your Social Security number in the search tool. You'll be able to see if you qualify as a class action member and can file a claim.
