This is the first app that I found that… the process to combine two files onto one page was simple.
This is the first app that I found that it will actually let you combine two files onto one page simply. The only bummer was that I had to convert the .pdf file into a .jpeg first before I could combine them together.

How are QDR paid out?
QDR OS are used to transfer money from a 401k (or other Qualified Plans) to an ex-spouse s IRA. With a QDR, the tax code also allows for the money to be distributed directly to the ex-spouse without paying the 10% penalty. Of course, ordinary tax rates would still apply.
Can I withdraw money from a QDR?
When an ex-spouse receives distribution of plan benefits pursuant to a QDR, he or she is responsible to pay the associated income tax. One key difference is that a cash-out distribution from a QDR is not subject to the 10% early withdrawal penalty.
How is a QDR paid out?
A QDR will instruct the plan administrator on how to pay the non-employee spouse's share of the plan benefits. A QDR allows the funds in a retirement account to be separated and withdrawn without penalty and deposited into the non-employee spouse's retirement account (typically an IRA).
How long does it take to get your money from a QDR?

Who pays the taxes on a QDR distribution?
A QDR distribution that is paid to a child or other dependent is taxed to the plan participant. An individual may be able to roll over tax-free all or part of a distribution from a qualified retirement plan that he or she received under a QDR.
What can I do with my QDR?
Take the money as a lump sum. Take the money as an annuity. Keep the money in the qualified retirement plan. Leave the money in the spouse's plan but retain the ability to invest your portion as the alternate payee as you choose. Move the money into a rollover IRA.
How long does it take to receive funds from a QDR?
A QDR is ordered by the court, but you company plan administrator process the plan documents. I usually take 60-90 days for this to be processed, but this is totally based on what your plan administrator schedule.
