duplicate pages I like being able to duplicate pages, that was super helpful. It was easy to add the text boxes where I needed them. The only trouble is in downloading the document to my regular google drive. Wish that was more straight forward.

How do I fill out a HIPAA release form? Provide instructions. Name the patient and individual authorized to use or disclose their PHI. Describe the information. Specify recipients. Some of the crucial information in a release includes: Name of the parties involved, i.e., releasor and releasee. Detailed information about the project.
