Make sure that the security settings allow form filling. (See File > Properties > Security.) Sometimes form creators forget to convert their PDFs to interactive forms, or they intentionally design a form you can only fill in by hand. If you can't type in the form fields, then the fields are probably not interactive.

Open the PDF file that you will use for the form. From the Forms menu, select Add or Edit Fields From the Add New Field pull-down menu, select Show Tools on Toolbar. Click Checkbox Tool. Position the cursor where you want the form field to begin.
Open your PDF document. Switch to Edit Mode. Wait for the Edit toolbar to appear. Select the Text Box icon. Click on the page that you want to add the Text Box. Remove place-holding text and enter desired text in the box.
There are two ways to select the Insert pages tool: 1. On the Home tab, in the Pages group, click Insert. In the Open dialog, select the document to insert. Click Open. In the Insert Pages dialog, select from the available page range options, and specify where the pages should be placed within your file. Click Insert.
