An easy method of filling out and sending documents. Problem with sending document due to 'Bugs' in the system It took three tries to send off a document and I still do not have a confirmation that it was send nor d I have a date in which the document would be received. The concept is good as I can go to the website and complete a document and send it off. Still some bugs

Download the template. The first step is to download the bookmark template by clicking the green Download button on the right. Open the template. Next step is to open the template in Microsoft Word®. Design your bookmarks. Now it's time to let your creative juices flow! Print!
Navigate to the page you'd like to bookmark. Press Command + D or click Bookmarks at the top of the browser window and select Add Bookmark from the drop-down menu. Name the bookmark and select the folder in which you want it saved. Click Add.
Select text, a picture, or a place in your document where you want to insert a bookmark. Click Insert > Bookmark. Under Bookmark name, type a name and click Add. Note: Bookmark names need to begin with a letter. They can include both numbers and letters, but not spaces.
Create your template. Divide the top left square diagonally from the bottom left corner to the top right corner. Fill in the triangles. Cut out your shape. Use your template to make your bookmark. Fold the shape. Form the bookmark. Decorate your bookmark.

Punch a hole in your bookmark. Fold a 3-4 inch piece of ribbon in half, and stick both ends through the hole. Before pulling all the way through, stick the two ends through the loop of the ribbon and pull tight, so the knot slides down to the base of the bookmark. Trim ribbon edges at an angle.
