Exactly the form that I required. Repeated editing was necessary for approval of recipient and permitted by PDF filler. Direct email was most helpful to pre-empt time delays. Anticipate emailing for initials & signature - hope it works out.

To use olive oil as a dried-on paper remover, soak a corner of a paper towel in olive oil, then spread the oil generously over the affected area. The paper stuck to the furniture should absorb the oil within half an hour or so. After that, wipe the loosened paper and the oil away with another paper towel.
Examine the paper and glue quality. Glue that adheres to strong quality paper will be easier to remove. Heat the glue. Use a hair dryer or heat gun to warm up the attached paper items in the scrapbook. Use scissors or a credit card to remove glue. Apply a solvent or acetone with a cotton swab to break down the glue.
Soften the glue with steam from a kettle or pan (place on a wire rack above a steam pan, taking care not to burn yourself on the steam). Place the item in a sealed jar overnight next to a cup or dish of rubbing alcohol (70 percent isopropanol/30 percent water) for further softening.

Correctly removing a poster includes physically removing the paper, disposing of the hard waste, and then blasting it with pressurized hot water so that any remaining glue residue is dissolved. In situations where residue still remains, a cleaning solution is applied to perfect the end result.
Soak a clean rag with warm water and wring it out until it's barely damp. Lay it over the sticky spot and leave it in place for 15 minutes to loosen the residue. Mix up to 1 cup white vinegar with a gallon of warm water in a mop bucket. Mop the floor with the vinegar solution.
