It is great! I would like instructions on how to save a blank form if you don't mind. I keep opening up the old form, then I have to delete data and replace it. Thank you!

Honestly- it is not straigth forward tool- you need to spend time and learn how to use it so it is not exceptionally user friendly. I found SignNow very complex!

Write a conversation with more than two people talking. ... Show the audience the name. ... Place characters in settings where their names get called out. ... Use phone calls and text messages to say your character's name.
When a name or a title appears at the end of a sentence, the name or title can follow either a comma or no comma. Again, both constructions are grammatically correct, but they have different meanings.
In a formal first reference to a faculty or staff member, use the person's formal first name and last name followed by degree (if applicable) and lowercase job title. If the individual routinely uses his or her middle name, include it.
Between a person's title and name (when the title comes first). When you write someone's title before the person's name, you don't need to use a comma in between: ... Do note, though, that when the title follows the name, it is set off by commas. (See #2 on the list of places you need a comma).
The basic idea is that if the name (in the above example, Jessie) is the only thing in the world described by the identifier (my oldest friend), use a comma before the name (and after it as well, unless you've come to the end of the sentence). If not, don't use any commas.
In general, you should italicize the titles of long works, like books, movies, or record albums. Use quotation marks for the titles of shorter pieces of work: poems, articles, book chapters, songs, T.V. episodes, etc.
First, state the name of the person being introduced to. This is the 'higher-ranking' person. Second, say I would like to introduce or, please meet or, this is, etc. Third, state the name of the person being introduced. ... Finally, offer some details about each, as appropriate.
Offer your name, ask for his name, and then quickly move on to the introduction. You can say something like, “I remember meeting you last year, but I can't recall your name. I'm Grace, and this is my sister Hazel.” If the person has good manners, he will state his name at this time.
Tradition dictates that if introducing a man and a woman of equal status (either in business or social situations) you speak to the woman first. Likewise, age takes precedence speak to the older person first. Speak first to a person with a title: Senator, Doctor, Reverend.
INTRODUCE YOUR FRIENDS RIGHT AFTER SAYING HI. ... INTRODUCE THEM BY THEIR NAMES. ... DESCRIBE YOUR FRIENDS BRIEFLY AND HIGHLIGHT THEIR STRENGTHS/ the BEST QUALITIES. ... POINT OUT SIMILARITIES IN INTERESTS/BACKGROUND/LIFESTYLE. ... INVOLVE EVERYONE IN CONVERSATIONS (FILL THEM IN) ... GIVE THEM TIME TO TALK TO EACH OTHER.
