Just needed access to a quick fillable PDF for SOAP notes when my office management software with builtin SOAP notes crashed. It is filling a need for the moment, doubtful I will continue. It seems like a nice program but the diagram on the SOAP notes is too cumbersome to mark with the given symbols on the chart as, so far, I have seen no way to actually mark the figure on the chart with the same symbols electronically. There are some symbols available, like the check, circle and line but they are not like the ones on the actual symbol chart. If there was a more expedient way to use those same symbols I might actually be very interested in using this.

A Phone Label can be applied to any telephone number in your Dialogue account as a way to categorize and identify the source of a call. For example, a Phone Label could reference a specific marketing campaign, customer type, or your dedicated customer service line.
Touch the label to the left of the phone number. Scroll down to the bottom of the list of labels, and tap Add Custom Label. Enter the custom label text and touch Save. Your custom label is now added to the list of contact number labels, and can be selected to label any phone number.
You can add a custom label to any number by tapping the Edit button while viewing the contact. Touch the label to the left of the phone number. Scroll down to the bottom of the list of labels, and tap Add Custom Label. Enter the custom label text and touch Save.
Click on contacts and then find the “+” symbol at the bottom and to the left of your contact list. You'll have an option of creating and labeling a new group. To add contacts to the group, click on “All Contacts” and drag/drop names on top of that group.
First go to the custom label that u want to delete through 'add a contact' (the + sign on the top right and then to the custom label) and hold down the label for few seconds & swipe left the label that u want to delete. A delete button (Red) will appear. And simply delete the label.

Apple support told me there was no way to do this, but there is, and it's easy. Then, go to settings > contacts > my info and tap the new contact you just made. Finally, go to contacts list from main screen, tap the info thing at the top, and delete the contact that it's set to.
