Write down your current name as the petitioner. Write down your current name again. Write down your address and place of birth. Confirm that you have never been convicted as a sex offender. Explain the reason for the name change.
Legal Document: a certified marriage certificate, final decree of divorce, or name change judgment from the court is needed.
