Fill it out! Overall, I love when this is available to use for forms I need to fax or email. I love the feature of being able to fill the form out online. Very easy to use and beats having to fill in by hand, scan and send. I am not quite sure how to do this myself and have to ask IT to assist in making the form fillable for me.

As long as you know the state where the inmate is incarcerated, you can use a website called vinelink.com (Victim Information and Notification Every day) to uncover the other details like inmate/offender ID, date of birth (DOB), race, gender, custody status, location and sometimes, even the scheduled release date.
Many correctional facilities offer a search option on their website to help you find an identification number. You will need to enter any identifying information you have (name, age, etc) and if the inmate is located in the database, you will be provided with that number. Use this number to complete the search.
To find the release date of a federal prisoner, locate the prisoner using the Federal Bureau of Prisons' online inmate locator (see Resources), then review the information to see if a release date has been posted. If not, call the facility.
