In practice, the vast majority of people with dissociative identity disorder do not obviously present as if they have 'multiple personalities'. But many symptoms may appear to be non-trauma-related, such as depression, substance abuse, eating disorders and anxiety.
The DSM-5 provides the following criteria to diagnose dissociative identity disorder: Two or more distinct identities or personality states are present, each with its own relatively enduring pattern of perceiving, relating to, and thinking about the environment and self.
Memory loss (amnesia) of certain time periods, events, people and personal information. A sense of being detached from yourself and your emotions. A perception of the people and things around you as distorted and unreal.
Dissociative disorders involve an involuntary disconnection with memories, emotions, perceptions, and behaviors, as well as with a person's own identity or sense of self. These symptoms can disrupt cognitive function and psychological wellbeing and can cause problems in every aspect of a person's life.
Cultivate empathy. Start by reminding yourself that splitting is part of the disorder. Try to manage your response. Remind your loved one that you care. Maintain lines of communication. Set boundaries. Encourage and support treatment. Take care of yourself.
Some people with dissociative identity disorder (DID) have very little communication or awareness among the parts of their identity, while others experience a great deal of cooperation among alternate identities. Currently, there is no empirical measure of integration versus fragmentation in a person with DID.
Integration refers to the mutual acceptance, empathy, and identification of all the different personalities. It is a process rather than a specific event. (Steele, 1989) Barriers are broken down and the personalities are able to come together.
