For pdfFiller’s FAQs

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
An affidavit is a form that allows you to provide additional information required to process your claim. Affidavits are sometimes used to prevent possible fraudulent activity on your account and ensure that only authorized users on the account are able to file a claim.
1 Print, fill out and sign the Sworn Affidavit & Proof of Loss Statement. 2 Photo copy your valid photo ID and handwrite your Claim ID number on the paper. 3 Fax both documents to 1-866-306-2268. Visit us online at phoneclaim.com/metro or call Asurion at 1-866-862-3397 for questions regarding the claims process.
Once your insurance claim is approved, you'll be told if Asurion is electing to repair or replace the device, if it's the latter, they'll ship it out to you overnight. It may be new, or it may be refurbished. A phone charger, battery, and a SIM card will be included.
Claims must be reported within 60 or 90 days of the date of loss, depending on your carrier. Second, when you receive your replacement device, you'll be required to return your damaged device with the prepaid shipping label that comes with the replacement.
It protects against device malfunction, damage, loss and theft. and carries a $49 - $199 deductible depending on the device. Verizon's deductibles tend to be a little lower -- most high-end Android phones won't hit the $199 deductible.
Simple. Be honest and return the damaged phone to Asurion, or they will charge you a couple hundred dollars as a penalty. HOWEVER, if you owe $1400 to AT&T they can blacklist your new replacement phone if you default on your bill.
You are required to return your damaged or defective device using the pre-paid shipping label provided with the replacement device. You will need to return the damaged or defective device within 10-15 days to avoid non-return charges. Check your wireless carriers' program specific information.
The 300 fee is to cover the cost of the phone they shipped you, not to let you keep the damaged device. Asurion owns that device, they will black list it, and you can be held liable for insurance fraud if you sell or use it. Just send it back to them, or cancel the claim and ship back the device they gave to you.
