Seems user friendly for the most part. It would be nice if the documents could be automatically opened in Drive without all of the signing in process each time

How do I get a Polish ID card?
Please go to oblate.gov.pl. Select the Get your ID card option and then Online. Log on to your equal account (Electronic Public Administration Services Platform). Select the commune office where you would like to apply and then collect the issued ID card. Fill in the form and attach your photo.

What qualifies as a national identity card?
Identity documents in the United States are typically the regional state-issued driver's license or identity card, while also the Social Security card (or just the Social Security number) and the United States Passport Card may serve as national identification.
What documents do you need to apply for a smart ID card?
Birth certificate. Certified copy of 1 of your parents / legal guardian identity document. Death certificate of certified copy if your parents are deceased. Proof of residence (where it's available) or provide residential address.
