The filler form was OK; however, it would have been better to know there was a cost and subscription up front before putting all the time in to fill out the form. Especially when it was shown as "FREE"

Awesome! If there is a way to add your logo I could not find it... then again I didn't look either so...this might not be accurate. So easy to use. extremely user-friendly. The benefits are that you get to create any type of form you need and customize it to fit your needs. You cannot ask for anything better than that.

For pdfFiller’s FAQs

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Physical credit cards have spaces within the card number to group the digits, making it easier for humans to read or type in. So your order form should accept card numbers with spaces or dashes in them.
Find the number located on the front of your card. Your credit card number should be either printed or embossed in raised numbers across the front of your card. This is usually a 16-digit number, although it may be anywhere from 12 to 19 digits.
A credit card number is the long set of digits displayed across the front or back of your plastic card. It is typically 16 digits in length, often appearing in sets of four. Sometimes it can be as long as 19 digits, and it is used to identify both the credit card issuer and the account holder.
If you ever want to see whether a credit card number is valid, add every other digit starting with the MID and multiply them by two. Then take each undoubted number and add them to the doubled ones. If you end up with a number divisible by 10, then the credit card number in question is real and valid.
If you have a fake credit card number, you can use credit card number generator for free at www.CCardGenerator.com. In this way, you will be able to generate up to 999 of random credit card numbers. They are already completed with name, address, expiration date, and the 3-digit CVV or security code.
For Visa, MasterCard, and Discover cards, the code is 3 digits and is found on the back of the card. For American Express, the code is 4 digits and is found on the front of the card.
Visit americanexpress.com/confirmcard. Enter your credit card number and the four-digit ID on the back of the card. Enter any requested personal information to verify your ID.
First digit is 4 for VISA, 5 for Mastercard, 6 for Discover/Diners Club, 3 for American Express/Diners Club (those are shorter than 16). Also, first 6 digits for Visa and Mastercard are code numbers for the issuing institution.
American Express cards start with either 34 or 37. Mastercard numbers begin with 5155. Visa cards start with 4. And so on.

You might be able to find the number in an online statement when you go into your online account. It's very unlikely they will also put your card's 3-digit security code, since that's only put on the back of your card (it's used to verify that your card is in your possession).
Get the Last Four Digits of Card from Chase Site Log onto the Chase credit card site at Chase.com. Scroll through the list of your accounts until you find the new one (likely at bottom of list). Once your new Chase card is approved the account will show up on the Chase account portal.
Digits 2-6: Make up the bank number. Digits 7-12 or 7-15: Represent the account number. Digits 13 or 16: Is a check digit.
On a Visa, Mastercard or Discover card, it is a three-digit number on the back of the card, to the right of the signature panel. With an American Express card, the code is four digits on the front of the card, above and to the right of the card number.
