I like to use PDF filler.com because I can fill the entire form on the computer, except the places where I need to sign it. Error in my ways!
I was unaware that I had used the service in the past which voided out my free trail. I then subscribed again and was instantly billed for a full year. When I realized my mistake, I reached out via instant message to the support staff. The service was stellar at best. They assisted me promptly and corrected my dispute which was impressive to say the least and speaks to the integrity of this service.

What elements indicate an accurate and complete medical record?
A medical record is considered complete if it contains sufficient information to identify the patient. Support the diagnosis/condition. Justify the care, treatment, and services. Document the course and results of care, treatment, and services. And promote continuity of care among providers.
How do you make sure medical records are accurate and complete?
Ensure medical record entries are clear and readable. Never squeeze words onto a line or leave blank spaces. Never erase, write over, ink out, or use whiteout on an entry. Never add anything unless you write a separately dated and signed note. Always indicate the date and time of an entry.
