So surprised and happy to find this VA… So surprised and happy to find this VA application online and available to convert to a filler style form. Printing the document by-hand was painful and re-dos were tedious but with your program I was able to complete and annotate (where needed) the application in a legible, presentable manner. Choose the Open Math Input Panel command in the Insert Equations group of the Datatype tab in Word. Write the equation in the MIP writing area. Once you've confirmed the accuracy of the equation, tap Insert to place the equation into Datatype Desktop.

Type y and press ENTER to insert a new line into Datatype. Select Align Center from Datatype's Format menu. Insert the Upwards Arrow from Datatype's Arrows palette and then press ENTER. Type “-x”.
CTRL + H puts you into superscript mode. CTRL + L puts you into subscript mode. CTRL + J assigns both a superscript and a subscript.
