trying to find the right form that was not already filled out. Also you need to add the ability to erase to the lowest membership level. You can do a couple of things such as restrict the number of new forms per month, offer a 1-5 form only fee, or only a 1 month membership fee. In the past 3 years I've used this service only twice now as I am not a business.

The 11-hour rule states that within the 14-hour window, a property-carrying driver can drive a maximum of 11 hours after 10 consecutive hours off duty. Passenger-carrying drivers can drive up to 10 hours after eight consecutive hours off duty.
706 miles. With the new rules you can drive 11 hours.
11 hours in one day You can't drive for more than 11 hours in one 14-hour period. After that, you have to stop and rest for at least 10 consecutive hours. The 70-hour rule If you work for a company that is in daily operation, you cannot drive more than 70 hours within any eight-day period.
In a nutshell it simply states that you cannot work more than 14 hours in a 24 hour period and cannot drive more than 11 hours in a 24-hour period, without taking a 10-hour break. You have 14 hours to be on duty from that moment on, before you are required to shut down for your 10-hour break.
As a general rule, it's safe to drive for no longer than eight hours a day, taking breaks of at least 15 minutes every two hours. This means you can safely drive for around 500 miles, not taking into account external factors such as slowing for tolls, traffic, travelling with children, and tiredness.
Each duty period lasts 14 hours, which is known as the 14-hour rule. Drivers can drive for up to 11 hours during the duty period. However, after driving for eight hours, the driver must take a break of at least 30 minutes.
The 14-Hour Rule When a driver comes on duty after taking at least 10 consecutive hours off duty, he has a 14-hour window to complete his driving for the day. Although you cannot drive after the 14th hour, you can be on duty to perform other work-related duties. A violation can only occur while driving.
The 14-hour rule prohibits the driver of a property-carrying commercial motor vehicle, operating in interstate commerce, from driving after the 14th consecutive hour after first coming on duty following 10 consecutive hours off duty.
