this makes filling out medical forms sooo much easier. I can fill them out and email them back without needing to print them and rescan or fax them back. Thank you for this wonderful software ! So this being a followup review I can tell you I still love it as much as I did before. I was able to use it to fill out medical forms and also a form for a winter retreat. The only drawback to this PDFfiller.com is the cost.

Below is a list of the most common customer questions. If you can’t find an answer to your question, please don’t hesitate to reach out to us.
Broadcast TV or Radio Program Begin with the title of the episode in quotation marks. Provide the name of the series or program in italics. Also include the network name, call letters of the station followed by the date of broadcast and city. “The Blessing Way.”
Name of person interviewed. Year of interview (in round brackets) Title of the interview (if any) (in single quotation marks) Interview with/interviewed by. Interviewer's name. Title of publication or broadcast (in italics) Day and month of interview, page numbers (if relevant)
List the interview by the name of the interviewee. If the interview has a title, place it in quotation marks. Cite the remainder of the entry as you would other exclusive web content. Place the name of the website in italics, give the publisher name (or sponsor), the publication date, and the URL.
Start with the title of the episode you want to cite in quotation marks, then the program or series name in italics, the station name, call letters of the station, the city and the date it was broadcasted. Finish with the publication media e.g. Television or Radio.
Instead, a personal interview should be referenced as a parenthetical citation. For example: (J. Smith, personal communication, May 17, 2008). If you would like to include a personal interview as part of your APA reference list, then include the interviewee, the date of the interview, and the type of interview.
Published interview from publication: Last Name, First Name. Interview with First Name Last Name. ... Published interview from Radio/TV program: Last Name, First Name. ... Unpublished interview: Last Name, First Name.
Use the following structure to cite the online transcript in MLA 8: Author's Last name, First name. Title of the Article or Individual Page. Title of the Website, Name of the publisher, Date the resource was published, URL.
Citing in the Bibliography Start the bibliographic entry with the title of the transcript in quotation marks followed by a period. Follow the title with the program name and broadcaster of the transcript. For example: All Things Considered and NPR. The name of the program should be put into italics.
Bottom Line: When citing yourself, in whichever style you are utilizing, cite in-text citations to identify yourself as the author. On your Works Cited Page (MLA) or Reference List (APA), identify yourself as the author using the format for an unpublished paper (or published, if you have published it!)
Cite published conference proceedings like a book. If the date and location of the conference are not part of the published title, add this information after the published proceedings title. Last Name, First Name, editor. Conference Title, Conference Date and Location, Publisher, Date of Publication.
