A completed Application for a Social Security Card (Form SS-5). Your certified name-change document, such as your: Marriage certificate. Divorce decree. Proof of your identity, such as your: U.S. driver's license. U.S. passport. Proof of your U.S. citizenship, such as your: U.S. birth certificate.

If your true full name isn't matching on your Birth Certificate, Passport, Social Security Card, or Driver's License (or State ID), then you should also bring a Name Change document (such as Decree Changing Name, Marriage License, Divorce Judgement, Adoption Order, Naturalization Certificate, etc.)
If your name has changed, you MUST visit a Secretary of State facility to obtain a corrected Illinois driver's license, corrected title and vehicle Registration ID card. Please bring your driver's license and title and a copy of the marriage certificate or other court document.

In Alberta, you do not have to legally change your name after getting married. You may assume a married last name.
