At first, I didn't understand what I needed to do first. I filled out the form I pulled up and then submitted it, but was sent to a page to choose which plan I wanted. When I realized I needed to choose a plan, I chose one, but then I had to fill out the form all over again! Then I submitted it, but the form again, but it wouldn't appear on my list of filled forms. My list only lhad a previous form from 7/15/2015. I chatted with a representative and she helped me get my second version on my list. This rep was extremely patient and nice, and did all she could to help me. I'm very happy I spoke with her.

Open the settings on your phone. Scroll down and tap General management. Tap Date and time. Tap the toggle next to Use 24-hour format.
Open your phone's Clock app. Tap More. Settings. Under “Screen saver”: Switch to analog or digital: Tap Style. Choose Analog or Digital. Dim for dark environments: Tap Night mode.

First, pull down your phone's notification shade and tap on the gear icon to take you to the Settings menu. Next, scroll to the bottom of the list and select the option for System. And finally, choose Date & time. The last step is to switch the system clock to a 24-hour format.
Also commonly called “military time,” that's where the afternoon shift switches to 1200 to 2400, taking care of those pesky a.m./p.m. concerns. And switching your phone is easy. Just go to Settings>Date & Time and check “Use 24-hour format.”
To change the clock format, visit the General section of Momentum's Settings, scroll down to the Clock Format option and then click 12 hours or 24 hours to the right of it. When you switch to the 12-hour format, the AM/PM unit will display for a moment and then fade away to keep the interface clean.
Click on the Windows 10 Start button and select Settings. Click on Time and Language. Next, click on the Change date and time formats link (See image below). On the next screen, click on Short Time and pick h:mm TT from the drop-down choices.
