As good as DocuSign Allows you to create pdfs that with blanks in order to easily fill out. There are no free versions, so you pretty much just hope that a business has taken the proper steps to acquire this and utilize it.

Wage Record Count In what are called “wage reporting” States, employers are required to report what are called “wage items” or “wage records.” This report is a listing by Social Security number of all persons who received pay during the quarter along with their total wages.
The Total Subject Wages in Item G on the DE 9C are wages subject to Unemployment Insurance (UI), Employment Training Tax (ETT), and/or State Disability Insurance* (SDI). The FT utilizes the PIT Wages and PIT Withheld information to verify an individual's wages and withholding.
Subject wages are the compensation amount an employer uses to calculate the amount of unemployment Insurance, state disability Insurance and paid family leave benefits an employee should receive.
Taxable Wages vs Subject Wages are monetary compensation given to an employee or independent contractor for work done. The amount that can be taxed is an employee's gross pay — i.e., before taxes. Once these wages are taxed, that person's pay becomes their net pay.
Gross Pay is the total amount of wages paid to your employee before non-taxable pay, such as expense reimbursements or mileage reimbursements, or non-taxable deductions are taken into account. Social Security Subject Wages = Gross Pay minus Section 125 pretax deductions, up to the Social Security wage base limit.
States generally require employers to file quarterly reports listing all employees and their wages during the reporting period. The wage-reporting laws don't require employers to report wages or earnings for non-employees or independent contractors. Employers often pay unemployment taxes when filing wage reports.
The IRS considers unemployment compensation to be taxable income which you must report on your federal tax return. State unemployment divisions issue an IRS Form 1099-G to each individual who receives unemployment benefits during the year.
The amount of unemployment insurance you get is based on your salary plus your tip income. Remember, tips are taxable. We can't say it enough, Sean, because the more tip income you report, the more unemployment insurance you can get, up to the maximum amount allowed in your state.
