prepare your base document, and save. then save as onto your computer desktop, and rename once downloaded onto your computer desktop. reuse the base document over and over for different clients.... works great and gave me the exact form i was looking for.

Can I change the name on my phone contract?
If you've bought a Pay monthly contract for someone else, in order to take your name off the contract and put their name on it, the new owner of the contract must submit to their own credit check. This is because the direct debit payments will be coming from their bank account.
Can you switch names on a phone contract?
You can give your mobile phone, phone number and contract to another person or a company that will continue to use the service and accept legal responsibility for paying the bills. We call this changing ownership of your phone. We also call it changing ownership when you switch from a prepaid service to a monthly plan.

Transfer to a friend, colleague or family member To change the account holder name on your mobile account and transfer it to a new owner, give us a call when both you and the new account holder are together. To transfer the account: call 150 from your EE phone. Call 07973 100 150 from any phone.
Can you take out a phone contract for someone else?
It's your phone, so you have all the choices. You can pay off the contract and cancel it or even report it stolen and have the phone and sim barred. Unfortunately you cannot transfer it into someone else's name though.
How do you transfer a cell phone plan to another person?
Go to att.com/tobr. Select Transfer of Billing Responsibility. If you use your ID to manage more than one account, select the wireless account you want to transfer. Choose the numbers you want to transfer.
