I found all kinds of IRS forms, but, not 2018 W2s. Finally I gave up after 2 days and emailed support. They responded immediately with form. It was awesome. I was shocked. What Should You Include on a Contact Page? Ideally, a contact page should include both an email address and a contact form for visitors to fill out. You may also choose to include a business address, phone number, or specific employee/department contact information.
Great contact forms typically: Are easy to find, so a visitor can quickly get in touch with you. Explain why someone should contact your business. Describe how your business can help solve the visitors' problems. Include an email and phone number so visitors can get in touch with you on their first attempt.
Form design principles to follow (the 4 Cs of good form design) Put labels above your fields, not as text that disappears within the field. Stack fields vertically when there are more than two. Make it obvious which fields are required. If there are more than 6 options for a form, use a drop down menu.
Create HTML Create a <h2> tag for the title and add a <div> element with a class "container". Create a <form> element and add an action attribute with a "/form/submit" url. Create four <label> elements with the following id attributes: "fname", "lname", "mail", and "country" and "message".
Historically, the standard answer has been that 3 is the optimum number of fields you want on your form to maximize your conversion rate.
