You should notify a person first there is a fee in case they have no money to pay your fee or do not wish to pay when you can print these forms out from many sources and fill in by using a pen.

Late I am and was because of my health, and the contractor didn't sign the W-9 form which I had waited for which he finally emailed it to me.
