I like everything I have tried with the exception of the fact that I cannot download a fillable form to my desktop. I wanted to make a form that I could pull up quickly on my computer when I have a new patient intake. It only allows a link and too many steps to make this a good alternative to the paper and pen format I am currently using.

I love this tool
I love this tool. I shared it with my mom who still has to fax things and I did not want her leaving her home to fax at other businesses. I think a video on how to create more fillable boxes would be helpful. Although I figured it out, it took me about 30 minutes. I can imagine for even less tech savvy people it could take longer.

I needed to use the service for an application
I needed to use the service for an application! It was only needed once. The day I was going to cancel it due to no longer needing the service the money had already come out of my account. What is split bill lading?
Split bills of lading are bills issued for part of a cargo originally shipped under a single set of bills of lading. They are most common in the bulk trades, e.g. cargo interests may wish to split a cargo covered by a single bill of lading under which delivery can only be made to one party, between say three receivers.
What is a split shipment?
A split shipment means that cargo moving via air does not arrive at a single flight, but is instead distributed among two or more flights. With these factors in mind, the airline may split a shipment in order to plan a safe flight. This is more likely to happen with a large shipment.
How are shipping costs split?
Find out if shipping is flat fee or is by weight. If it's flat fee, split it evenly between all people. If it's by weight, divide total shipping cost by number of items to get the shipping cost per item, then multiply shipping cost per item by how many items a person ordered.
Why did Amazon split my order?
When shipping to a single location, Amazon pays to disperse your inventory across the country on their dime. If it is not cost-effective for Amazon to do this, they split your shipping plan into multiple shipments. There are two ways to avoid this: Pay to force a single shipment.
