Add as many signers as you need and enter their email addresses. Move the toggle Set a signing order to enable or disable sending your document in a specific order.

You do, however, need to talk to your physician, who will sign your DNR. In some states, adult witnesses or a notary public must also watch you sign the order. A prehospital DNR order won't have any effect unless emergency medical personnel see it before they begin CPR.
Because it is a real-time medical order, a DNR would typically not be in place for a healthy person who would likely wish to be resuscitated.
It is an emergency rescue technique that was developed to save the life of people who are generally in good health. NOTE: If you do not have a DNR orders, health care providers will begin CPR in an emergency.
Use a form provided by your doctor. Write your wishes down by yourself. Call your health department or state department on aging to get a form. Call a lawyer. Use a computer software package for legal documents.
A DNR or Do Not Resuscitate request is usually made by the patient or health care power of attorney and allows the medical teams taking care of them to respect the patient's wishes. In the U.S., CPR and advanced cardiac life support (ACLs) will not be performed if a valid written “DNR" order is present.
DNR orders must be dated. Depending on the state, orders may expire after a certain amount of time or there may be a deadline for the physician to follow-up. Even if a DNR order doesn't expire, a particularly old order may prompt caregivers to revisit the decision with patients.

Do not resuscitate order A DNR is a request not to have CPR if your heart stops or if you stop breathing. You can use an advance directive form or tell your doctor that you don't want to be resuscitated. Your doctor will put the DNR order in your medical chart. Doctors and hospitals in all states accept DNR orders.
The doctor writes the DNR order on your medical chart if you are in the hospital. Your doctor can tell you how to get a wallet card, bracelet, or other DNR documents to have at home or in non-hospital settings. Standard forms may be available from your state's Department of Health.
A notary public can witness the signature of the person filling out the form in lieu of two witnesses. If two witnesses are present, the form does not need to be notarized. Note: In Section D of the OOH-DNR form a notary cannot witness a competent person making an OOH-DNR order in a non-written manner to a physician.
The DNR must be signed by the patient or by the patient's legally recognized health care decision maker if the patient is unable to make or communicate informed health care decisions.
A DNR is typically for patients who are terminally ill, suffering from an end-stage condition or a persistent vegetative state. Because it is a real-time medical order, a DNR would typically not be in place for a healthy person who would likely wish to be resuscitated.
A do-not-resuscitate (DNR) order can also be part of an advance directive. Hospital staff try to help any patient whose heart has stopped or who has stopped breathing. They do this with cardiopulmonary resuscitation (CPR). A DNR is a request not to have CPR if your heart stops or if you stop breathing.
