Cut the paper trail and faxing Instead of faxing documents, I just get them emailed. The documents are more clear, it's fast, and I can sign and fill in easy without needing to fax it back. I love the signature button. This is so easy to use. Uploading and downloading is all I do now. My fax machine is obsolete. You have to download it to your computer from email, then upload it to pdf filler - at least that's how I do it.

You will need to ask your doctor for an itemized bill. Your health insurance company will need you to attach the original itemized bills to the claim form. Get Your Claim Form. You will need to contact your insurance company to obtain a health insurance claim form or download a copy from their website.
Use this form to submit a health benefit claim for services that are covered under the Blue Cross and Blue Shield Service Benefit Plan. Submit a separate claim for each patient. Complete the form following the instructions on the back. (You can fill the form in electronically or complete it by hand.)
These contracts invariably include a requirement that the provider submit all claims for reimbursement to the HMO/insurer within a specified number of days (typically 90 or 180 days) after the date of service, and that failure to submit the claim within the required time period will result in denial of payment.

Simply put, a claim is what a doctor submits to your insurance company, so they can get paid. It shows the medical services that were provided to you. Typically, your doctor or provider, especially if they're in your plan, will submit the claim for you.
Claim. A request for payment that you or your health care provider submits to your health insurer when you get items or services you think are covered.
Most states require insurers to pay claims within 30 or 45 days, so if it hasn't been very long, the insurance company may just not have paid yet. It may take a couple of weeks to get the claim approved and processed and for your provider to get paid.
The statute of limitations on hospital bills varies between states but is generally three to six years. It applies to payments due, not the billing itself. Hospitals can therefore continue to try and collect payment outside the limited time.
You can claim tax exemption only up to Rs. 15,000 per year. If you have spent more than Rs. 15,000, the excess amount is taxable (even if the company reimburses the excess amount). If you do not produce any medical bills to support your claim for reimbursement, the entire Rs. 15,000 is taxable.
If you offer employees reimbursement for medical expenses without a formal plan, reimbursements are treated as taxable income. Employers should avoid reimbursing employees directly for medical expenses or pay for their expenses directly.
Under final regulations, beginning in 2020, employers may offer individual coverage health reimbursement accounts (Chris) that reimburse employees for individual health insurance premiums, subject to satisfaction of several conditions.
Medical expenses incurred on self, spouse, children or completely dependent parents & siblings can only be claimed for exemption. These medical bills need to be submitted only with the employer. No reimbursement can be claimed at the time of filing the income tax return.
You may deduct only the amount of your total medical expenses that exceed 7.5% of your adjusted gross income. You figure the amount you're allowed to deduct on Schedule A (Form 1040 or 1040-SR).
The executor can choose to deduct accrued (as-yet-unpaid) medical expenses, along with any medical expenses paid before death, on the decedent's final Form 1040. Final medical expenses can easily exceed the percent-of-AGI threshold, especially when death occurs early in the year before much income has been earned.
