Owners of properties in the State of New York that pose an actual or potential risk of contamination to the water supply system should complete and submit the GEN215B, Form for Report on Test and Maintenance of Backflow Prevention Device form to the NYC Department of Environmental Protection. This requirement applies to properties with any of the following facilities: bakeries, car washes, greenhouses, warehouses, pressure tanks, water storage tanks, etc.
What is the Form GEN215B for?
This form is the summary of the test performed by the Certified Backflow Prevention Device Tester in order to determine whether the operating conditions of the system (meaning the backflow prevention devices) comply with the requirements of legislation. These tests are needed to prevent water from flowing back into the city’s drinking water supply.
Is Form GEN215B accompanied by other forms?
This particular form should be submitted within thirty days of installation of the backflow prevention device and filing the GEN-236 form with the NYC Department of Environmental Protection. Without the GEN-236 form, this GEN-215 form cannot be filed.
At least once a year this form must be completed and filed as the report of the mandatory annual testings.
