Who needs a Form AF-IMT-1288?
Air Force Form 1288, Application for Ready Reserve Assignment is to be used by the Air Force Reserve Recruitment Center for persons wanting to be a member of the reserve. This form is to completed by a soldier wanting to be transferred to the reserve as a Traditional Reservist or an Individual Mobilization Augmented.
What is Form AF-IMT-1288 for?
Request for Ready Reserve Assignment must contain current personal information to complete processing. Based on the information provided, an applicant’s personal records will be identified by the recruitment center. After the application is considered by the recruitment center, an applicant may join a Reserve unit, usually near applicant’s home place, and train with this unit for one weekend a month and perform two weeks’ training each year.
Is Form AF-IMT-1288 accompanied by other forms?
The last three applicant’s performance reports, AF Fitness Assessment and a Preventative Health Assessment completed forms should be filed as a necessary accompaniment for the Application for Ready Reserve Assignment.
When is Form AF-IMT-1288 due?
The form can be submitted at any time the need arises. All the mentioned above accompaniments must be completed within the last 12 months before filing the 1228 form.
